Wegovy now comes in six dose strengths: 0.25 mg, 0.5 mg, 1.0 mg, 1.7 mg, 2.4 mg, and, as of March 2026, a new 7.2 mg HD option. The first five follow a step-up schedule every 4 weeks, reaching the standard 2.4 mg maintenance dose around week 16 to 17. The 7.2 mg HD dose isn't part of that initial climb.

The full Wegovy doses chart

What’s new about Wegovy HD

Wegovy HD was FDA-approved in March 2026 as a higher-dose option layered on top of the existing Wegovy lineup, not a replacement for it. It’s intended for people who’ve tolerated the standard 2.4 mg dose for at least 4 weeks and, in their provider’s judgment, would benefit from additional weight loss. In the trial that supported approval, people on 7.2 mg lost an average of about 20.7% of their starting body weight over 72 weeks, compared to roughly 17.5% on 2.4 mg and 2.4% on placebo.

The higher dose came with a notable increase in one particular side effect: dysesthesia, an altered or unusual skin sensation, was reported by about 22% of people on 7.2 mg compared to 6% on 2.4 mg. Overall safety was otherwise consistent with what’s already known about this class of medication. A single-dose pen for Wegovy HD was expected to launch in the U.S. in April 2026, so availability may still be catching up at some pharmacies.

You don’t have to reach the top of the ladder

There’s no requirement to progress all the way to 2.4 mg, let alone to HD. The 1.7 mg dose is itself FDA-approved as a maintenance dose, meaning some people stay there long-term if that’s what works for their body and goals. Whether to step up further is a conversation between you and your provider based on how you’re responding, not a fixed destination everyone is expected to reach.

Why the step-up schedule exists

Semaglutide, the active ingredient in Wegovy, has a long half-life of about a week, meaning it takes roughly 4 to 5 weeks for blood levels to fully stabilize after each dose change. The 4-week spacing between steps isn’t arbitrary; it’s built around how long the medication actually takes to reach a steady level in your system. Moving faster than the approved schedule mainly increases the risk of severe nausea and other GI side effects rather than speeding up results.

Who Wegovy is for

Wegovy is FDA-approved for adults with obesity, or adults who are overweight with at least one weight-related health condition such as high blood pressure, high cholesterol, or type 2 diabetes. It’s also approved to reduce the risk of major cardiovascular events, like heart attack and stroke, in adults with established heart disease who also have obesity or overweight. A licensed provider determines eligibility based on your BMI, health history, and any related conditions, not a chart alone.

Missed dose guidance

If you miss a dose, take it as soon as possible if it’s within 5 days of your scheduled injection day. If more than 5 days have passed, skip that dose and resume your regular weekly schedule at your next scheduled day rather than doubling up to make up for it. Consistency matters more than perfection here; a single missed dose handled correctly generally doesn’t derail overall progress.

Common side effects

Nausea, diarrhea, constipation, and reduced appetite are the most frequently reported side effects, especially in the days following a dose increase. These are usually mild to moderate and improve as your body adjusts to each step. Less common but more serious risks include gallbladder problems and pancreatitis, which is why ongoing check-ins with your provider matter throughout treatment.
